               I.         Assumption of the Risk and Waiver of Liability Relating to COVID-19

In order to participate in Little Stars Sports Club and Facilities Management LLC. (herein referred to as Little Stars) events and activities, you must understand and be aware of the risks involved and agree to accept responsibility for your/ your child(ren)’s informed and voluntary participation. Little Stars cannot guarantee that you or your child(ren) will not be exposed to COVID-19 while participating in Little Stars events or activities. By signing this agreement, I acknowledge and agree to the following:

I, acknowledge the potential risks associated with participating in activities at Little Stars during the COVID-19 pandemic, including exposure to COVID-19 that can result in illness, injury, permanent disability, and death. I assume all risks related to exposure to COVID-19 and agree to comply with all health and safety protocols put in place by the relevant Abu Dhabi Government Authorities and Little Stars.

To the fullest extent permitted by law, I myself, my spouse, my child(ren), and on behalf of my/our heirs, assigns, personal representatives and next of kin, I release and waive any and all claims against Little Stars, its owners, its employees, and its volunteers related to exposure to COVID-19.

              II.         Assumption of Risk, Consent and Waiver of Liability

In order to participate in Little Stars Sports Club and Facilities Management LLC. (herein referred to as Little Stars) events and activities, you must understand and be aware of the risks involved and agree to accept responsibility for your/ your child(ren)’s informed and voluntary participation. Gymnastics and other sports activities offered by Little Stars may involve height and rotation in a unique environment and as such poses risk of injury. Little Stars has implemented necessary safety measures without removing the inherent nature of the sport. As such:

I certify that I/ my child(ren) am/is/are physically fit, and I know of no physical reason, illness or medical condition that would preclude my/ my child(ren)’s participation in Little Stars events and activities. I hereby declare that I now have and will continue to provide personal health and accident insurance for myself/ my child(ren), covering injury treatment which I consider adequate for protection.

I understand that during participation in Little Stars events and activities, I/ my child(ren) may be exposed to a variety of hazards and risks, both seen and unforeseen. I understand and accept that the activities that I/ my child(ren) will engage in at Little Stars involve inherent and other risks. These risks can result in varying degrees of physical harm, risk of minor or serious injury, illness or even death. I also understand that it is my responsibility as parent/guardian to familiarize myself. my child(ren) with any safety guidelines outlines by Little Stars.

I acknowledge that it is my sole discretion to allow my child(ren) to engage in Little Stars events and activities and will assume all risks, both inherent and other risks and accept responsibility for any personal property damage and loss and for any degree of physical harm my child(ren) may suffer. I also agree that this agreement is binding not only to myself but my next of kin, heirs, executors, administrators and assigns.

Being fully aware of these risks, I hereby give consent for my child(ren) to participate in all and any Little Stars events & activities, and I accept all risks associated with this participation.

To the fullest extent permitted by law, I myself, my spouse, my child(ren), and on behalf of my/our heirs, assigns, personal representatives and next of kin, I release and waive any and all claims against Little Stars, its owners, its employees, and its volunteers for any liability, costs, damages, medical expenses, long-term care, emotional distress or compensation that might incur as a result of my/ my child(ren)s’ participation in Little Stars activities whether caused by the act, omission, negligence or fault, active or passive, direct or indirect of any of the Little Stars owners, employees, volunteers and/or representatives. I understand that Little Stars is not liable for personal injuries to the applicant’s family members and their guests as they use the facility at their own risk.

             III.         Medical Release

I fully understand that Little Stars staff members are not physicians or medical practitioners of any kind. With that in mind, should I/ my child(ren) become ill or injured while partaking in Little Stars events or activities, I hereby grant Little Stars employees, volunteers and representatives the authority to render temporary first aid to myself/ my child(ren) or obtain the emergency medical attention they deem necessary including calling of an ambulance or transportation by a Little Stars staff member, representatives or volunteers to a healthcare facility or hospital.

            IV.         Image and Video Release

I am aware that individual and group publicity photos and videos are taken from time to time of Little Stars events and activities and in consideration for my / my child(ren)s participation, I grant permission for my / my child(ren)s likeness to be used in Little Stars publicity, media usage including but not limited to social media, training tool and videos and/or advertising without notification or compensation.

               V.         Payment Policy

Class sizes are limited. A seat is only allocated and confirmed upon receipt of a full payment. Tuition is non-refundable and non-transferable. Payment shows your commitment to the entire term allowing us to effectively schedule coaches for your child’s class.

​

 

              VI.         Make-up Policy 

Due to the limited capacity of classes, students can attend class only on the day/s that have been confirmed for them at the time of registration. Missed classes are forfeited.

 

The club does not close for public/school holidays during the term, all classes will continue as scheduled unless informed otherwise by administration. Make-up classes cannot be provided if a family chooses not to attend a class during a holiday period when classes remain operational.

             VII.         Responsibilities

Parents need to be on-time to pick up their child(ren). Many children get anxious when class is over, and a parent is not there to pick them up. A waiting area is available on the premises, if you choose to leave the premises for the duration of your child's class, please make sure to arrive 10-15 minutes prior to the end of the class for pick-up.

Little Stars is not responsible for items left on its premises. We maintain a lost and found box and apply every effort to recover your missing item but cannot guarantee your item will be found.

             VIII.         Class assignments

We reserve the right to move a student to a different class if the age or skill prerequisites outlined for the booked class are not met. Our priority is to ensure that each student is placed in an appropriate class for their level of ability and development. If such a situation arises, we will contact you to discuss alternative options. Thank you for your understanding and cooperation in ensuring the best possible gymnastics experience for your child.

 

I hereby declare my legal responsibility for myself' and my child(ren). I myself, my spouse, child(ren) and on behalf of my/our heirs, assigns, personal representatives and next of kin agree to all of the above terms stated in this agreement.

 

I understand that by electronically signing this document it will bind me to all terms and conditions of this Agreement and that the electronic reproduction of this Agreement will serve as an original signature in any Court of the United Arab Emirates.
